Old Age Security identification card

The Old Age Security (OAS) identification card is sent to you when you receive OAS benefits. It is not necessary to apply for one.

To replace an OAS identification card that is lost, stolen or damaged, call or write to Service Canada.

Send mail requests to the nearest Service Canada regional office. Be sure to give your full name, your social insurance number (SIN) and your mailing address.

Send mail requests from outside Canada to the Service Canada regional office in the province where you last resided.
